FIND A FACILITY

On the homepage, under the section “locate a facility” there are two ways to find health care facilities.

  1. Screen capture of the Locate a Facility section of the main pageTo get a list of facilities with the name, address, phone number, owner’s name and other information, click “Search by Facility Type.” On the page that comes up, click the arrow next to the “Facility Type” box, scroll down and click one of the facility types. (If you need the definition of a facility, click “Glossary” on this same page.)

    For a statewide list of these facilities click “Submit.” For a list by “County” click the arrow next to that box and choose the County then click “Submit.” This is usually the best way to find facilities that serve your area of the state. You can also search by City or Zip Code by entering information in those boxes, though that may give you less choices than using the County search.

  2. To find facilities within a certain distance click “Search by Proximity” on the homepage. On the page that comes up, enter the street, city, state, and zip code from where you want to start your search. Click the arrow next to the “Facility Type” box, scroll down and click one of the facility types. Choose the distance then click “Submit.” This will give you a map as well as the names and street addresses of the facilities you chose within the distance you chose.

To get a map and step-by-step driving directions for a particular facility, click the yellow “Map Location” arrow next to a facility’s name.
